HIPAA Notice of Privacy Practice
The law requires that we protect the privacy of your Protected Health Information (PHI) and that we give you a Notice of our legal duties and privacy practices with respect to PHI. PHI contains information that may identify your past, present or future physical, mental health conditions or healthcare services. This Notice explains how we can use PHI in the course of providing contractual obligations as a business associate to your healthcare provider or other covered entity.
SwiftReach Networks (SRN) does not sell, rent, or lease its customer lists to third parties. Data provided by customers is the property of the customers of SRN. Customer-supplied data includes contact information and may include other necessary data as per the customer’s specifications. Customer-supplied data is kept confidential and only used as per customer contract.
SwiftReach Networks is committed to a secure, safe and private environment for all customers’ data up to and including PHI in its electronic form (ePHI) consistent with the Health Insurance Portability Accessibility and Accountability Act of 1996 (HIPAA) and other state and Federal laws as applicable.
Authorized government requests for data should be directed to any customer whose data is being legally sought although this does not absolve SRN of direct requests authorized by law related to legal proceedings, law enforcement, Workers Compensation, Military, National Security and Intelligence Activities and for the Protection of the President.
SRN’s data network is designed to protect customers’ data from unauthorized intrusion and consistent with SRN confidentiality and privacy policies and procedures, all employees are regularly trained and tested on confidentiality and privacy issues.
We reserve the right to change this Notice and to make the revised Notice effective for all health information we create or maintain. Upon request we will make the revised Notices available to you. The revised Notices will be posted and available at 14 Industrial Avenue, Suite 4 Mahwah, NJ 07430.
